No written requirements for the Co-op Office.
Month Two / Three
At approximately the midway point of the work term (weeks six to ten) the Co-op Coordinator will conduct a site visit. Prior to the site visit the person who is supervising and / or mentoring the student will be asked to complete an Employer Mid-term Evaluation of Co-op Student Web Form.
The visit will consist of a 20-25 minute joint meeting with the student, a representative from the company (usually the person(s) supervising and / or mentoring the student), and the Co-op Coordinator. During that time, the Supervisor and / or Mentor will review the evaluation with the student and Co-op Coordinator.
Once the joint portion of the meeting is finished, the Co-op Coordinator will spend 20-25 minutes one-on-one with the student.
Month Four
Each student must complete and submit a Work Term I Report approximately half way through the fourth month of the work term. A deadline date for submission of these reports will be announced each term.
It is also the responsibility of each student to ensure that an Employer Final Evaluation of Co-op Student Web Form is completed during the last week of the Co-op placement. Where possible, the person who completes the form (usually the student's Mentor or Supervisor) should review this submission with the student. The deadline date for submission of this web form will be announced each term.
Please note: the Co-op Office requires all forms prior to submitting a grade (P/F) for the work term.
